The first stage of dental implant surgery

The traditional two stage dental implant surgery requires two separate visits to the dental clinic. During your first visit, the surgeon opens the gum tissue, bores a hole in the jawbone and inserts, actually screws the implant into the prepared hole, strictly following the process described in your comprehensive personalized dental treatment plan. The implant is placed under the surface of the gum, so your gum can therefore be closed back up, covering the implant.

TThe place of intervention is then left to rest until complete osteointegration is achieved. Osteointegration is the process when the bone tissue surrounds and actually bonds to the specially treated surface of the titanium dental implant. This process takes place during a period of 2-3 up to 6-9 months. The healing time depends largely on the position of the dental implant in the mouth and also of personal aptitude, general state of health and on the way the dentist's instructions and guidelines are followed.

 

Note, that you are never left toothless in the healing period. You are supplied with temporary crowns or bridges, or your original dentures are modified to fit the new form of your gum, always keeping in mind that these temporary prostheses solutions should never become a risk factor. The temporary prosthesis is never connected to the implant, it does not put any pressure on the newly inserted implants. It is the neighboring teeth or the untouched part of the gum that takes over the support function of the prosthesis temporarily and you will be able to continue your life at least the way lived before the intervention. These solutions are always planned to protect the implants as much as possible.

 

The insertion of a dental implant in the jaw bone has a stimulating effect to the bone structure, and by the time the healing period is over, not just your implant will be steady enough to support the crown or bridge but also your bone structure will be stabilized.

The second stage of dental implant surgery

It is during your second stay, generally 3-6 months after the first, that the process is continued with the next phase by the fitting of the dental abutment into the dental implant. A minor incision is made on the healed gums to access the implant. In some specific cases during the first phase already a healing abutment is fitted into the implant that barely penetrates the gum level. This already forms the gum and the need for opening the gum up again can be avoided.

 

With the abutment in place the impression is taken of your jaw, which is usually taken digitally in our dental clinic. The dental laboratory prepares the type of prosthesis you have agreed on with our dentist in an extremely short period of time. Dental crowns and dental bridges made of any dental material, like zircon, porcelain or porcelain fused to metal is rapidly prepared at our fully digitized dental laboratory partner, which is then cemented to the abutment by the dentist, giving you the final desired new tooth or teeth.

How is the immediate loading method different?

Is one stage enough?

There is another method for placing a dental implant, which is the one stage method, also referred to as the immediate loading method. The major work using this method is generally said to be all done in one stay. It certainly sounds interesting and appealing. 

 

The dental implant in this method is placed without surgically opening the gum to have access to the jawbone, instead the hole is drilled directly through the gum. Then the special one stage implant is inserted, screwed into the prepared hole. It is usually an emphasized advantage, that this is a less invasive intervention.

 

An impression of your jaw is taken after the intervention and sometimes already the permanent new crowns or the dental bridges are prepared and cemented in place right away. In other cases, temporary crowns or bridges are fitted so in this case there still needs to be a second visit after total healing is achieved when the temporary prosthesis is replaced by the permanent dental prosthesis.

 

Constructional differences of the dental implants

The dental implants used for the immediate loading method are not identical to those used for the two stage method. One major difference is that the immediate loading dental implants are much thinner, therefore more dental implants need to be inserted to withstand the same powers as in the two stage method. The other major constructional difference is that the implant screw is built together with the abutment so after the intervention the fixed abutment part of the implant sticks out of your gum, which therefore needs to be covered right away at least by temporary prosthesis.

 

The risk of failure in the immediate loading dental implant method

Your new permanent or temporary crowns or bridges, are therefore directly connected to the newly inserted dental implants, the new prostheses are actually cemented to the abutment of the dental implant, which obviously makes it very difficult if not impossible for the patient to eliminate all physical strain on the new dental implant. So this is why the most commonly encountered problems of the immediate loading methods are related to the premature overloading of the dental implants which induce the micro movements of the dental implant. This unfortunately directly influences the osseointegration process, and increases the chance of the loosening of the dental implant, which directly prevents osteointegration and gives place to a possible infection.

 

The process of osteointegration requires time and during this period of several months all movements, including micro movements of the dental implants should be totally avoided.

 

This single stage method contains a much greater risk of both immediate failure and also a delayed failure than the traditional two stage method. As described most of the times you need a second session for the immediate loading method as well, in order to change the temporary prostheses to permanent ones, where rapidity, as one possible advantage of this method, is therefore given up.

 

Only upon the fulfillment of very specific circumstances does the use of this method give you a fair possibility of success. According to studies primary stability absolutely needs to be achieved in order to have the least of chance to real success. This means the candidate needs to have enough quantity of good quality, that is dense bone structure, where the implant is stable enough already before the commencement of the osteointegration process which only takes fully place if the implant is left intact without any movement at all, not even micromovements should be allowed to disturbe the healing process.

 

But just how can a dental implant be kept from even the slightest movement if your crown or bridge you eat and chew with is directly supported by your new dental implants?

As a result of this obvious contradiction the  Dental Implantology Association of Hungarian Dentists has issued a statement in which the immediate loading implants are not recommended in Hungary even if it is available and actively advertised by some dentists. 

 

Most of the times the immediate loading technique is performed by dentists who are not oral surgeons, only dentists who completed the training offered by the dental implant manufacturer. They are therefore prepared for the insertion of the dental impalnts but possibly not as prepared for any complications that might arise during an intervention or the healing period as you would expect from an oral surgeon.
